Administrative charge. I would email them and bill them a charge back of a $20.00 processing fee to cover the time it takes to write out your check.

There have been lawsuits over some taxes in some states that had legally lapsed years ago and should not have continued to be charged... If there are taxes and surcharges you question are valid, most states have a regulatory agency that oversees traditional and cellular phone communications. If asked, they have to provide you with that contact.

Yes, it is ridiculous, but at least demand someone KNOWLEDGABLE explain them to you and question them all. If possible demand a supervisor do so. I'd say go into a store to do so, but they are even less likely to know what the charges are... At least that has been the case with AT&T...

You have a choice of four major carriers and a dozen MVNO's. If I don't like my service and pricing, I change my provider.

... same as your "administrative charge."

I own the phone and it's a crappy pay-as-you-go plan, but I really don't like to use telephones much.
