Unless they suffering from something an mental health affliction that affects their perception of reality (like schizophrenia), it's the hatred and enjoyment that they get out of making others suffer that compels them to do what they do.

They totally understand what they are doing is wrong, and they would not want someone murdering someone that they care about. But they have convinced themselves that either what they are doing is just, or don't care because they like the power trip.and get pleasure from it.
