I'm pretty sure he wouldn't put up with the discomfort and pain especially ones in his feet (spurs can hurt terribly/pinch nerves).

Oh and unlike so many other Americans, I'm also pretty sure that he can afford the surgery.

What I see is that either his shoe lifts are pitching him forward as has been said and/or he has some sort of neurological problem throwing him off. For instance... leaning forward is commonly seen with dementia patients.

From the side, the lean makes his face more in line with his midsection, whereas standing straight would cause his gut to stick out way ahead.

The lean looks freaky in full body view, but when photographed from the waist up or if there something obscuring his legs (podium, etc.) then the profile view is closer to what you'd see for a slimmer man (like Trudeau in the tweet in the OP)
