The president's schedule and movements - especially when he leaves the WH - are timed down to the minute. An OTR is something that was not previously on the schedule. It can be an activity that was actually vaguely planned in advance but for some reason wasn't committed to the schedule, perhaps because it allows the president to spontaneously at the last minute decide if he wants to do it - for example, going to church or going shopping or stopping at a favorite barbeque place.

It could be something they decided not to alert in advance for security or some other reason.

Or it could be something that just came up, like an unexpected rush to the hospital.
