The fastest known is the Helios probe, at some 43 miles per second, launched in the mid-'70s.

The second-fastest is a metal cap (basically, a thick manhole cover) that was welded to the top of the shaft that contained an underground nuclear bomb during a test in the 1950's.

There was a high-speed camera pointed at the plate when the bomb went off. Despite the speed of the camera, the cover was caught in only a single frame during its flight.

Based on shutter speed and the frames per second, the cover had a minimum velocity of 41 miles per second.
