With certain exceptions carved out.

For example the insurance company needs enough information to confirm the patient is covered and identify what was done before they (you hope) provide compensation for covered services.

Or if the patient owes money and you need to refer them to a collection agency. Or when necessary to comply with law enforcement needs. Not that either of those are probabilities to factor in.
