5. How do we get people to ask for help when they need it?

It is incredible difficult to admit that one needs help. Sometimes we can't even recognize when we need help, and we deny to others that we need it, even in the face of all other indications.

Sometimes getting help means acknowledging criminal behavior. Sometimes it can mean just as much stigma as criminal behavior even if asked for in advance. Think what you would do if faced with such rage that you were about to harm someone. Who would you go to? Who would they go to when they consider themselves overwhelmed? How long would it take to get 'authorities' involved? Remember the subject is asking for help before he goes off the deep end. How many authorities can help without a crime being committed? How many authorities have to call other authorities to warn of the possibility of harm?

It's a nightmare.
