1. Cheney has heart trouble

When somebody has a heart attack in a hospital, orderlies scream for the "crash cart" containing the defibrillator (it's too heavy to carry).

2. Look at his heart

That asshole has a serious heart condition and has had I don't know how many heart attacks. He's had open heart surgery, and he has an implanted automated defibrillator. "Crash Cart" refers to the cart on which all the medical equipment used for someone in cardiac arrest is stored. It can be wheeled to the bedside during an emergency at the hospital.

Calling him VP Crash Cart is a sweet way of wishing him the "best."

3. The crashcart refers to two things:

A crashcart is the mobile machine (defibrillator) that is used to shock the heart back into a stable working rhythm. ("Clear!")

Cheney has had a bunch of heart attacks, and even has a defibrillator inserted into his chest with wires to his heart. If his heart goes into a bad rhythm, the little implanted device will give him a little shock to stable his heart.

4. The nicknames of Dick Cheney......

"Crash Cart". As in that cart with all the heart attack stuff on it at the hospital.
