The problem started when the wrong voter-instruction booklet was stuffed into 7,400 ballot envelopes, McPherson said.
When the printing company caught the mistake, workers pulled each envelope by hand. All of the materials in the envelope – the booklet, return envelope and absentee ballot – were separated into piles. But some of the different ballot styles were put in the wrong pile, she said.
