I can only say; frightening.

It looks remarkably authentic. And remarkably disgusting.

No idea if it's real, but his apparent age in the picture looks like right around the time when he was running around drinking and blowing a lot of coke, so it seems possible.

Has that washed out too-cyan color temperature of late-60s, early-70s color prints. Those things didn't age well.

I ran it through some filters in Gimp, and I see nothing to indicate any editing to the scanned original.
