Public High Schools REQUIRED to give STUDENT INFO to U.S. MILITARY
If you or your friends have kids in public high school,
you should know that, under the Leave No Child Behind Act,
all public high schools are now REQUIRED to report student
private info to the U.S. military....

3. that's been so for some time. i think you can opt out, though...
....or at least you could. But you had to take the step; otherwise the info is presumptively going to the military

5. There is also an opt out option.

I just signed one for my child who will be entering HS in the fall. The district had no specific form called opt out. But they sent home a paper asking parents for permission to give their childs info out. I declined and signed for my child and wrote next to the option that under no circumstances is she to have any contact with the military, especially recruiters.
