1. If you're in Idaho, Utah, Arizona, or New Mexico

I've just signed up for Cavtel for phone and DSL ($50/month for unlimited local, .05 cents/minute long distance and dsl)

5. You might also consider
A local ISP, if you don't do a lot of mobile computing. There are any number of local companies that would be more than happy to provide you with internet service and access for a charge comparable to the big boys.
