It can also mean "hand by hand" - literally...and *hand to hand* can also be translated to "mano alla mano."
Wouldn't Spanish be "mano a la mano?" Or is there an idiomatic phrasing?

I like to say main à main. Mainly because anything French seems to bother neocons so much.

The literal translation, of course, means hand to hand. But since it's a figure of speech, it's not the literal translation that's important. The context in which it's used means man to man, or more PC, person to person. As in, "let's you and me step outside, mano a mano."
