Is it to be worn as a shirt? Or is it a lingerie thing? And does it hurt like hell?

it doesn't hurt at all. women used to suffer in corsets because they wore them all day, every day, as tightly as possible. worn more casually they're rather comfortable (although it's weird to not be able to bend at the waist)
