... or at least the part that's left along the abdomen (the "tail", the part we eat), is on the OUTSIDE, where it cooks thoroughly. I don't think pathogens are a concern.

fried pig intestines, pigs feet, blood sausage, spleen, etc.
I have had stomach stew in Germany. When I go to foreign countries I just point at the menu rather then try to translate things. Makes it more fun. I got this stew. It had tiny bits of something in it - smooth on one side and fuzzy on the other. I didn't want to know what it was. It was delicious. I waited until I got home to find out it was stomach stew.
As for the rest listed above - I've tried them except for brains and spleen. Pig's feet are easier very drunk. I got one fork full of intestines down my throat, but when the fork started on the second bite, my mind told me that the next one was going to come right back up.

It's just about all dirt anyway. Their digestive system has already taken out all the "good stuff" and left the grains of dirt they ingested. That's why it's sometimes a little gritty. Keep in mind, this is a simple, stripped down animal. It ain't all the stuff we have going on in our digestive system.
I'm a zoologist specializing in crustaceans, AND I'm from N.O... so I should know. Of course, if it makes you sick, it doesn't matter if the shrimp itself stands up and says "Don't worry old chap. It's just a few micro-grains of sand. Have at me!"
