High Tension is supposed to fit your need (?) ..folks at work LOVED it, I have not seen it as I said.

I thought Haute Tension (bizzarely called 'Switchblade Romance' here) was excellent up to a point. Then you'll tear your hair out at the lunacy of it all (well I did). Still, some really good gore done the old-fashioned way.
"Saw"'s good if you haven't seen it -- a bit like "Se7en".
"House of 1000 Corpses" has its moments; think a more twisted "Texas Chainsaw Massacre".
And a real oldie (well 1999) - Takashi Miike's "Audition" delivers.
For some quality gore - "Guinea Pig 2: Flower of Flesh and Blood" is great, but don't expect any shocks. It's just a psycho slowly dismembering a woman for 45 minutes!
"Terror Firmer" delivers big style in gross-out gore, but it's not a horror by a long way (I just want people to see it!)
If you want serious, nasty as fuck, hardcore, Fred Vogel's "August Underground" & "August Underground Mordum" will do it for you. (You won't find it in Blockbuster though.)

is the scariest movie I've seen in a long time. It's more suspenseful/creepy than gorey, though. Unfortunately, I think it's currently in that limbo stage where it's out of most theaters but not yet out on video/DVD.

I went to see it tonight...definatly worth the money. It is very graphic and gory. The tag line "Oh yes, there will be blood" is an understatement. The only issue I had was that it was short - only 93 minutes.
The only reason you might not want to see this is if you haven't seen "Saw." You can follow "Saw II" if you haven't seen "Saw," but you might not appreciate some of the details as much, and the ending might seem a little messed up.
Saw was one of my favorite movies of 2004.
