5. Wow... great marketing tool, easy enough to figure out who you are...

if you answer all those questions honestly... first name, age, ethnicity, zip code, number of cavities, whether you've been divorced, whether you have been convicted of a felony, health history of your family, and NOT only that, but AN E-MAIL address... I think it would be pretty easy to track someone down.
