It's based in perception of the one that states it (as a negative) or in the perception of the one that receives it. So, if the person that hears it, percieves of it as a positive, then the power is removed. For example, the expression "fat dyke" relies on two things: 1. that the person suggesting same, understands and can judge who or what is "fat" 2. that the person suggesting same, is allowed to suggest that "dyke" when coupled with "fat" is negative.
So
"Fat" is relative. Does the person care?
"Dyke" might be slang, and used improperly, but it's a fucking compliment. A compliment, to me, for my gay friends. It suggests someone that's comfortable in their own skin, regardless of how others see you. To me, sort of like the word "fag" you know. You can say it all you want, but it doesn't change anything, except in how I see you. But...and I say this with the utmost caution...if you use these words in a hateful, violent fashion, my fist might well render you speechless.

when my mom and her friends use it with each other, it doesn't bother me; when someone else says it to or of my mom, I confess that a blind rage is pretty sure to follow.
Sometimes, the difference is all in who it's coming from.

I never eat chocolate, so I might as well have just shot up. If Margaret Cho says it, it's okay cuz she's cool. If someone like Elizabeth Hasselbeck said it, I'd have to kick her scrawny ass.

mailing her calling her "A Fat Dyke Gook Chink." But she did not see the insult with fat dyke. Cho is cool because she admitted going down on a woman and really liking it, so she thinks it's neat. She also said she likes dykes who look like John Goodman.
