What info should I put on my dogs tags?

What info should I put on my dogs tags?
I am going to get some tags for my dogs today, what info should I include? I just want the basics so if someone finds the dog they can help him/her to get home.

1. Just the dogs name and your phone number(s)
That's what I put on my cat's tags. I figure they'll call me if they need to, but if they want to keep my cat (and I pray that never happens) at least they'll have the comfort of being called by their original names!

2. Occasionally I find an escapee dog,
usually on a windy day when gates blow open. After calling the telephone number, if no answer, I've taken the dog to its address to see if the gate to the back yard is open. If the latch is securable, I put the dog in the yard and leave a note or call later to let the person know that the dog got out. That's one scenario where it helps to have your address.
(If my bossy Border collie/heeler girl wasn't so snippy. the dog would be welcome at my house/yard until I could contact the human!)

No, really, I think the animal's name and your phone number should usually do it. Address optional.

5. all the relevant phone numbers
and the address. People say not to put the animals name on there since it might be helpful to someone with less than pure motives.
