they do it every year -- the top 25 "fattest cities" and the top 25 "fittest cities". The judging criteria are things like how many health clubs in the city, how long of a commute, how many parks and open spaces.
So, it's quite a misnomer to say "fattest city". It would be better to say "Cities where it is relatively difficult to live a healthy lifestyle" But they need a gimmicky marketing device like "The fattest city", and it seems to be en vogue to make fun of fat people, so there you have it.
