She was actually related to her husband (1st cousin of his by his father's estranged brother--husband's father had a huge, eleven-child family and no one even knew the estranged brother had ever produced any kids). But she fell in love with her future husband, even though she knew he was her cousin. After she had her name legally changed, she pursued him romantically and they eventually married. But she has lived in fear of discovery for decades, knowing that if anyone actually wanted to check, they could uncover the evidence of her deception.

Somehow they meet in this state (him awake but insane, her asleep but sane) and then try to work out a satisfactory relationship, hampered by unfortunate complications including: he is cured of his madness but then literally drives her insane; she awakes is so insanely busy that he falls asleep from exhaustion.
