"this" was trying to drive a custom machine tool through a steel clamp, and "that" was accidently starting a mill in the middle of the program, instead of the beginning.
In both cases it turns out that custom finishing tools do not work really well when large chunks of steel are in the way. They tend to change shape in a sudden and unfixable manner.
<sigh>
I got written up.

you still have your job. I was fired once for smashing the front end of a $80,000 truck. That light pole just jumped out in front of me lol.

I forgot to load one side of a pallet for the horizontal mill. The Komet custom tool that has inserts for a 24mm bore, facing the bore, and putting a chamfer on the inside, I instead tried to ram through one of the little (but surprisingly solid) hydraulic clamps on the pallet.
On Wednesday, I drove a Komet OD chamfer tool down on a work piece on the vertical mill... without letting the plunge mill shave off a half-inch or so from the end of the work piece.
$250 for an endmill? Musta been a fairly large one!

with a six-insert Komet tool, but I was really fast on the E-stop. I tried to do with the Komet tool what is normally done with a much-shorter 3/8" endmill. 1,000 inches per minute straight down. Amazing what the sight of sparks can do to your reaction times... :scared:
The tool survived, but the vice moved, requiring about five hours for my boss to re-calibrate.

...by realizing that Texas and Oklahoma really shouldn't be forty milliseconds apart. It wouldn't have been a big deal but for the customer opening his first site in Canada and seeing 150 msecs to Oklahoma. My coworkers were still blowing him off when the account team came to me.
Turns out there's fiber in our network that takes a scenic tour of the southeast, and after a few days of prodding, I've gotten my customer moved to a direct Ft. Worth - Oklahoma City path, and I'm betting that we'll get him 15 msecs instead. I've saved us the revenue for his existing dozens of sites, plus the new capacity he'll buy in Canada.

We got equipment that go from 10,000-50,000 apiece. The real expensive one, the 50,000 one, I scheduled to train on. Its such a complex piece of scientific equipment that I do fear breaking it when I am learning on it...:scared:

One's about $140,000 each (we have two of those), one's about $200,000. About half of the tools used for any one part number are custom tools, so there are tens of thousands of dollars worth of tools in each machine at any one time.
The hydroformers are even worse. A single tool weighs 1,000 pounds, takes 100 hours to machine from two solid slabs of steel, and costs about $40,000 all told to make!
