My assistant's wife and daughter must call him at least 20 times a day |

They seem to be incapable of doing anything without his input. And his cell ringtone is annoying.
I'm seriously considering imposing a 'no cell in the office' rule; it's getting that bad.

1. "No personal calls" should be enough, |

and that is a pretty standard rule I think. Maybe he needs a reminder.

2. I don't mind a personal call here and there |
|
I need to take them too. But he drops everything to take their calls, unless it's a meeting. They're abusing the policy.
A reminder is definitely in order. Good thing his evaluation is coming up.

4. Yeah, everyone has them now and then... |

the Put Your Damn Phone On Vibrate Because Nobody Wants To Hear The J-Lo Ringtone You Bought rule should be put into effect.

A ringing cell phone not only distracts you, but it also distracts all of your coworkers that are within earshot of the phone. It doesn't matter if it's a purchased ringtone or the standard ring that comes with the phone, it's annoying, distracting, and rude.

the woman across the room from me with the loudest voice on the planet (ie her whisper is my normal speaking voice volume) Her husband calls that many times for the smallest things it is maddening. It is a line that rings all the time (or should) but she is tying up the line talking about what to make for dinner or when to cut the grass?

7. That's it; it's all for ridiculously small things |
|
Like, the dog needs a bath and something came in the mail today. All things that can be taken care of when he gets home.

6. Make a rule and enforce it |
|
or else he can stay home all the time and they won't need to call. Its his problem, not yours. You might offer him alot of time off to fix it.
