I would place number 3 as number 1:
3. My most hated expression has to be "to be honest". What does it mean? Are you normally dishonest then? To my shame you might even catch me saying it.

My ex uses this all the time and it really pushes my anger button.

I'm not a big fan of the giving more than 100% sayings either.

1. "It is what it is."
For whatever reason, I hate that phrase.

Also, I too dislike the misuse of the word "literally" when the thing being said isn't actually being taken word-for-word literally. Literally.

7. Yes, my most despised phrase.
I first heard it from a co-worker who was involved with one of those groups like EST (can't remember the acronym). So, sometimes it is what it isn't?

9. Yeah, I despise that "No offense but.." line
that ALWAYS procedes something nasty..I also hate the giving more than 100% and as a sports fan I hear that all the time from atheletes....

14. I've gone to a few 'communication' seminars
and it's always pointed out that when you use "but" in a statement it is only what comes after the 'but' that people focus on and that's probably because that is what the person speaking thinks is really important (or true) too.

Let me add:

"Exception that proves the rule." (doesn't make sense, anyway, it's sposed to be exception that PROOFS the rule.)
