4. The "Rat" (rathskellar) in Boston was below street level.
well, the music part of it was, the restaurant was upstairs.

Not even close. In part because everyone knew it was a dive. Liverpool's The Cavern has the historical chops, but the problem is that in its day it was just a club, a haunt. People going there and playing there didn't know it was a dive until years after the fact.
