Edited - actually, I was thinking of a different BB song.
They do use theremins occasionally, though.

5. Not really BB did not use theremin
Common mistake. They used a Tannerin which sounds like a theremin but gives spot on pitch control.

3. That part is being played by two mandolins.
One is playing the half notes and one is playing the arpeggiated eighth notes.

Both Ray Pohlman and Barney Kessel played mandolin on the session. (Kessel's was a custom built 12-string)

It sounds extra odd, because they are recorded pretty far off mic (with lots of reverb). They are also slightly out of tune, which adds to the dreamy effect.
