In this picture his arm is risen at similar height. I didn't feel like going through multiples pages to find one with both arms but that looks the same level to me as far as his right arm is concerned. Not saying this is McCain or isn't(looks like him to me) just that I'm pretty sure he can raise his arms at least to the level in the photograph.
