It was less obvious in the previous two debates, but I notice very clearly now the severe loss of muscle tone on the left side of his face, with the corner of the mouth drooping severly, and not animated at all. The left side of his face is nearly immobile. His facial expression is entirely asymmetric.

This guy has suffered a stroke. I suspect it happened back when he claimed to have choked on a pretzel and hit his head.
