And his hand is in a really weird position on her shoulder.

It looks to me like Bush's fingers are on top of her shoulder, such that his palm is sort of behind her shoulder where it can't be seen. Then there is someone else's hand coming in from the right of the picture (you can see the guy's chest but not his head - it's hidden behind the close-up figure in the right of the frame). Together, in the frame, Bush's fingers and this other guy's hand look like one bulky, strange appendage -- but it's clearly two hands.
