1. They broke at 2:30 AM Tuesday
And people should have been evacuated the Saturday and Sunday before.

... causing an ever-increasing leakage that washed out the buttressing embankment until the wash-out was extensive enough to allow a wide section of wall to give way. The 'break' took time - it didn't happen like a busted balloon. Part of the immediate emergency response should have been a full-bore inspection effort by the Army Corps of Engineers, using spotlights if necessary, as soon as the trailing eyewalls of the hurricane had passed and the winds dropped below 30mph. If I remember correctly, it could have commenced somewhere around 3pm local time.

5. Levees began failing on Monday during peak of hurricane
First news reports came late Monday. Eyewitness accounts of local residents are now coming forward, indicating that the breaches initially formed during the peak of the storm around midday Monday.
