5. Yes, it makes me nervous but seems to be an outlier.
At least the news seems to think Obama has the advantage in SD. I haven't seen any other polls.

It appears that Obama did well with them in North Dakota, Minnesota and Wisconsin, but every tribe is different.

10. What % of the state lives in Sioux Falls and Rapid City?
Obama will likely win those two cities and if the rural areas have very little population he will likely win.

Although they are not like many American cities - they don't have large African American populations. Not really fair to call the rest of the state 'rural' though. If you add in the populations of the top ten cities, that totals 47.7%. The rest of the state is probably rural and even in the urban parts, only four cities are over 20,000 in population - Sioux Falls, Rapid City, Aberdeen, and Watertown. The 10th largest city is Vermillion with 9,975 people.
