What will be the biggest reason for discrepancies between poll averages and the Nov. 4th results?

Poll question: What will be the biggest reason for discrepancies between poll averages and the Nov. 4th results?
Note: Half of these favor Obama, half favor McCain--and one is neutral. But ALL of them have been mentioned at some point in the last month or so, by the media, Nate Silver, etc--I'm just trying to take everything into account.

2. There's no "undecided" category for Presidential election RESULTS--undecideds either
don't vote, choose a major party candidate, choose a minor party candidate, or cast undervotes.
