How about discussing objections to particular policies rather than simply using derogatory names? Calling someone names says more about the speaker/ author than the individual who is the target of the insult. There are plenty of substantive issues with which to critique Bush and others on the right. Using our minds to develop thoughtful critiques will do far more to advance progressive politics.
If people want to be satirical, at least be intelligent and funny about it (like Jon Stewart, for example).

Caligula's nickname, because bush parallels Caligula in so many ways, it's amazing. It also makes fun of bush's habit of wearing shit-kickin' cowboy boots, like he thinks it makes him a real cowboy.
