Looks to me that Singapore (two-tier), Cuba (single-payer), and Malta (single-payer) know how to keep costs down, and Japan (single-payer) knows where to keep life expectancy up. Will this be another nail in the coffin of market theocrats? Probably not. :wink: But we can hope.

Which is why two tier systems seem interesting. Presumably the first tier is affordable and efficient and the second tier is, well, as much better as folks want to shell out more cash for.
