He probably doesn't even appreciate or realize he's the most despised single human on the planet, quite a dubious distinction.
If he were some sinister 'Dr. Evil' character in a bad movie, he'd probably giggle with delight at being the most hated person, but more likely he sits around fuming and fretting and wondering why nobody loves him.
What a wonderful place in history he has secured for himself, and aren't we oh so lucky to have him as our leader?

Just as long as they get attention of any kind they consider it wonderful. How they get it, who it hurts, what kind it is, is a matter of indifference to them as long as they get it.

... when it's actually the almost total absence of some internal sense of self. This is not a man that's capable of self-esteem, self-reflection, or conscience. His very sense of existence feeds upon the reactions of others, negative or positve. He's one sick puppy - the most extreme example of Narcissistic Personality Disorder I've ever seen (I'm NOT a professional.) It's quite possibly the absolute WORST mental illness/disorder that one could imagine in the office of the Presidency. Coupled with an obvious sociopath as Vice President and we have a nightmare of government.
It's pitiful (and laughable) that anyone would speculate on what he "feels" or what he "thinks" when it's clear that he's emotionally and mentally incapable of framing any 'thought' or 'feeling' in other than whether it feeds his pathology. This is much of why NPD is effectively untreatable - because the mere attention of treatment is itself a 'narcissistic supply.' He has absolutely no capacity for compassion or empathy - one might as well talk about flying like a bird or breathing water like a fish since it's no longer part of his make-up. In a just world, future editions of the DSM-IV (-V?, -VI?) would have his photo in the description of NPD.

The ones I ran across were effectively untreatable. If for no other reason than that they saw absolutely nothing wrong with themselves or their behavior even in the face of overwhelming evidence.

And he believes that:
The troops adore him.
The "silent majority" (a la Nixon) think he is wonderful.
Everyone who goes to church worships him.
The pollsters are all 'librul liars'--even the GOP ones.
The media is out to get him, except Faux, which, being fair and balanced, is only out to get him ten percent of the time.
And the odds are good that he is fed a steady diet of positive stories, though the poor bastards doing that research are probably working a helluva lotta overtime to even dig them out...

He is the most hated human in the history of the species! I truly believe there has never been a human being as disliked by his fellow humans throughout the globe as our worthless "president".
There are plenty of worse people, but not in such a prominent position.
Hitler? Maybe. But there were a lot fewer people in the world then, and I just don't see his actions as doing the world-wide damage that the chimp has managed (maybe because we stopped him?). Or is he the winner?
Are there any other contenders or is it basically just the two of them?
