Help, need info on FAA whisteblowers info and blaming errors on the pilots

hubby has a meeting on Monday about this issue and I can't locate it, but I know I read it about a couple of days ago, he said this would help him alot.

It was FAA whistleblowers here at Dallas Fort Worth, one came forward months ago and now another talking about mistakes made in air traffic control being covered up, there has been an investigation and now a pretty serious blow up just in the last few days
