but know enough about how such work to expect that they should be local (relatively) and initiated by local 'buyers.' They could happen here, and have done so to a small extent, but not likely to occur nationally, imo.

were referring to the prospects of small co-ops making arrangements with employers to provide coverage. And they be correct about that. Employers do not want to deal with a different insurance provider for each city they have employees in.
I work for a very large company (Verizon), and used to have insurance through George Washington U.'s health network, which was locally run and very responsive and convenient. They were dropped about ten years ago in favor of a smaller number of national providers.
This has nothing to do with what employers should do, just what any employer trying to run their business is likely to do.
On the other hand, the health care bill has an individual mandate. Those individuals might very well be a good target market for new local co-ops. So it might indeed work in another form, instead of or side-by-side with a public option.
