What is the derivation/origination of "thumb up my ass?"

What is the derivation/origination of "thumb up my ass?"
As in, "we did nothing but sit around with our thumb up our ass." As catchy a phrase as it is, it just seems like a school yard vulgarity which has been given more mileage than it deserves, though I admit I use it when I want to wake someone up.

5. It started in a restaurant.
A man noticed his waitress had here thumb in his soup as she set it before him.
"Hey", he exclaimed, your thumb is in my SOUP!"

She replied, "yes, I know, there's a sore on it and the hot soup makes it feel better."

He inquired with some anger, "Why don't you just stick it up your ass?"

She answered, "Oh, I do when I'm back in the kitchen."
