I had been having some problems in professional dealings and was somewhat hurt at being called a ball-breaker. (If you expect a man or men to do what they are supposed to, you hold them accountable and you happen to be female, this sometimes happens). One person who joked about it was a dear friend, so I took it to heart and REALLY did some self examination to see if I was being too rough on people.
Much self evaluation and still failing to see where I had been out of line, I called dear ol mom.
"Mom, am I a ball breaker?"
Long pause...
... ...
"Well, dear, yes, but only when it needs to be done."
So, I might just get me one of those HRC nutcrackers and put it on the mantle next to Mom's picture. Mom would get the reference. Maybe her shade would get a kick out of it.

I take the phrase to mean anyone who is overly persistent with you or trying to sell you a bad deal. Cartman uses the phrase in this context many times.
"Hi Larry, I got some foetuses to sell. How much can you give me?" (undecipherable answer) "Oh Larry, you're breaking my balls."

Ballbreaker and nutcracker have been in common use as an insult to strong, assertive, achieving-or just plain aggressive, controlling and bitchy-women for many, many decades preceding the South Park cartoon.
It's usually reserved for women who make men feel emasculated.
