2. There's no reason to do that now.
No one is paying any attention to bush.

I worry more that Cheney will resign for health reasons, and they will give the vice presidency to McCain. They might like to have him campaigning as an incumbent.

4. I don't think that would happen either...
the last thing McCain wants is to be linked to bush. That would most certainly seal his fate (as in defeat). The further he stays away from bush, the better for him (except for the fund raising money, of course).

6. Or they could have McCain very publicly refuse the position.
As a way to separate him from the Bush White House.

3. Bush will never resign...
he doesn't believe he's done anything wrong. Not to mention the hole that would blow in his legacy. He would have to have a serious come to Jesus moment before that would happen.
