Sociopaths who have gloried in the degradation and torture of their fellow man, particularly those whom law and culture have decreed to be inferior and therefore fair game. Also, imagine being picked up by these unwanted visitors with guns in the middle of the night, in front of your family, then tortured to give them names. How many "innocent" people can you imagine would be picked up & jailed for, actually, no good reason (because someone gave them your name just to stop the torture)?

Which is more plausible: that he did it to himself because he's that crazy, or that the guards/interrogators did it to him, then claimed he did it to himself?

It was claimed by whoever leaked the picture, and I tend to believe them. Why would somebody leak a picture like this and then protect the interrogators with a lie like that? If they were trying to hide something the interrogators had done, then they wouldn't have released the picture to start with.
