They're telling the stories of people who have struggled as a result of no or inadequate healthcare and she's grinning awkwardly at various points. At times because she's not paying attention and grinning at people she knows off stage. Other times grinning because the citizen speaking said "bankruptcy" when describing her potential circumstances and Pelosi while standing behind the woman mouthed the word "bankruptcy" to someone standing beside her while grinning awkwardly. Her awkward actions seem to trivialize (or at the very least distract from) the devastating stories being told and the general narrative.
