Vermont Medicare progam to serve as the model for feds

WASHINGTON — The Obama administration announced a new Medicare pilot program Wednesday to strengthen primary care services for patients and cut costly emergency room visits — by offering financial incentives to doctors.

Health and Human Services Secretary Kathleen Sebelius said the three-year pilot program would begin early next year and is modeled after a program being tested in Vermont.

Under the Vermont program, private insurers work with Medicaid to set uniform standards for so-called "medical homes" — a single doctor or practice that will track all the patient's required care. The model provides financial incentives for doctors to spend more time with patients and offer better care by coordinating with specialists. That program would now extend to Medicare.

Medicaid is the federal-state program that provides health insurance to the poor. Medicare is the federal program that provides health insurance to the elderly.

2. OK, I don't understand wha that means. I understand the
funancial incentives part, but what are they aupposed to be coordinating with specialists? Dost that mean the primary doc won't have to sent a patient to a specialist? I actually agreer with that. I lived the first 64 years of my life never having seen a specialist for anything! My primary doc handled everything, and I think these docs are far too eager to send you to a specialist now for some things they could well handle themselves

4. damn, can i have one?
can we all have one? this is a great idea. i have been down this- referred to a specialist and treated forever for something that the pcp would have taken care of right away. just to make the pcp's responsible to know what the hell the specialist is doing would be a big improvement.
