together with a spark from the cautery unit can be risky- but it is rare. It can also occur when alcohol based preps pool at the surgical site, and aren't allowed to dry, or wiped dry with a sterile towel.
If laser safety protocols aren't being used during laser cases it could occur.
Usually while working on the anus, a moist pack is inserted to prevent methane from escaping. The packing is removed at the conclusion of the procedure..
Again, surgical fires are very rare.

Flash fires in surgical theaters are still common enough to warrant extensive safety measures.
At one time, when ether was administered in open-circuit systems, they were a frequent occurrence. Switching over to (non-flammable) chloroform reduced this hazard quite a bit, but the extensive use of oxygen guarantees a heightened level of risk.

I feel for the woman's family. My mother is in a nursing home in Marion--hope she doesn't have to have surgery.
