and I enjoyed it - psychiatry - but too much politics in this state - greed is the major motivation for most of my colleagues and it is endemic, even reinforced, by the state system. My choice has always been to do what I want to do as long as I can do it ethically, and that is virtually impossible in this system.

I treat them with dignity and respect, which is rare in this field.
I don't like the paperwork or the politics. I don't like making choices for adults, I like helping them to make good choices.
I don't know how much longer I can do this. I don't know what I will do next. I have no money, no savings, and poor health.

As it effectively has me living full time in two different countries while having day to day responsibilities in both. US Customs knows me by name, a Delta Airlines pilot recognized me in a Wal-Mart. Fortunately for me my girlfriends employer is basically giving her the same treatment which makes it hard for her to be too tough on me but provides us both with abundant frequent flier miles.
