Why is this term used so broadly (and inaccurately?) Older history books do this a lot, but not so much any more. However it is still widely misused in common speech.

I think it is more a reference to the language spoken rather than national or racial derivation.

3. Yet it isn't applied to non-white English speakers. It seems, in the vernacular
to mean any white English speaker (even if she's Polish.)

8. I thought you were implying that I thought it was a slur.
It's a misidentification that some COULD find offensive (like the Irish.) But it not a slur. It's more like calling all East Asians "Chinese."
