Hospital confirms another fatality after medication error

An infant died while in the care of Children's Hospital after a staff member administered medication without an order from a physician, the hospital's medical director said Thursday.

Dr. David Fisher said the infant was in transport at the time, and the staff member administered the drug without permission in violation of the hospital's policy.

He said the child died around Sept. 17 while being transported to the hospital for a transplant, but the cause of death has not been determined.

Another patient, an adult who'd come to the hospital in life-threatening respiratory distress, fell severely ill after given medication that was wrongly administered, Fisher said. That person has recovered.
