If he can't lie flat without a whole lot of pain, get him to a shoulder specialist. Recliners are the best thing in the world for shoulder injuries and for recovery from shoulder surgery. And frozen black eyed peas make the cheapest and best non-drippy cold packs - reusable a nearly infinite number of times.
