Mr Serra, 68, was examined by doctors after the incident, which came as some of his supporters scuffled with campaigners for the ruling Workers' Party.
His campaign team said he was left feeling dizzy and he cancelled the rest of his activities for the day despite doctors giving him the all clear.
Mr Serra, a former governor of Sao Paulo state and candidate for the Brazilian Social Democracy Party (PSDB), condemned the attack, which happened in the working-class suburb of Campo Grande.
"This is the assault troop of the Workers' Party," he said. "You remember the assault troop of the Nazis? This is typical of fascists."
Brazilian media reported that the "heavy object" which struck him was a roll of tape.
