in the not too distant future (actually, now, as the book was written in the early 90s) the federal government of the United States has become completely inoperative. In response to its incapacity, FOQNEs grow up, which are basically like independent countries inside the US, distributed all over the place (like a McDonald's franchise). They confer certain benefits on their members/citizens, and provide things like a police force (subcontracted out to one of several police organizations), social programs (such as there are) etc on the citizen.
Of course, it's a nightmarishly dystopic world. Pretty much anarchy, and not very progressive at all. But in the face of a non-functioning government, I don't know that it's that far in our future after all.
